Attention Fall Athletes for the 2012-2013 School Year
Each fall athlete must fill in the packet that applies below and hand in to the nurse/athletic office by the dates listed. 
 
Sports Physical Packet (Part A & B)
Both parts must be handed in by July 30th
Physical Packet Part A
Physical Packet Part B
 
Re-eval Sports Physical Packet (Part A & B)
Both parts must be handed in between June 18th  and July 20th
The re-eval packet is for an athlete that participated at Phillipsburg High School during the last school year and handed in a physical dated after August 13th.  This packet will cover you until your next physical- a physical is only good for 360 days.
