Most forms are available in English and Spanish. If you are are unable to print them, you can fill them out and/or obtain copies at the Phillipsburg Education Center at 50 Sargent Ave, Phillipsburg, NJ during your Registration Appointment.

The Stewart B. McKinney-Vento Education of Homeless Children and Youth (McKinney-Vento EHCY) program provides students experiencing homelessness with protections and services to ensure they enroll in and attend school; complete their high school education and be positioned for success after graduation, so they may avoid poverty and homelessness as adults. Request/Release of Records:
This form allows parents/guardians to authorize the transfer of their child’s academic, health, and other school records when enrolling in or withdrawing from the Phillipsburg School District.
