Parent Forms and Documents

All form are available in English and Spanish. If you are unable to fill them out online or are unable to print them, you can fill them out and/or obtain copies at the Phillipsburg Education Center at 50 Sargent Ave, Phillipsburg, NJ.
 
Registration Forms for Attendance Based on Student Residency
Only one of these forms need to be completed according to the situation that best matches the student’s circumstances. Click here for more information on these forms
  • Section A - Domicile: Parent/Guardian's permanent residence/domicile is in the school district.
 
  • Section B - Affidavit Student: Student is living in the school district with someone who is not their parent or guardian.
 
  • Section C - Temporary Resident: Student is living with a parent/guardian who is temporarily residing in the school district.
 
  • Section D - Special Circumstances: Student's situation is not addressed by Section A–C(above), or any of the special circumstances, listed below, apply.
    1. The student is the child of a parent or guardian who has moved to another school district as the result of being homeless.
    2. A court order has placed the student in the home of a school district resident who is not the student’s parent or guardian.
    3. The student has been placed in the school district by the Department of Children and Families acting as the student’s guardian.
    4. The student’s parent or guardian:
      1. previously resided in the school district;
      2. is a member of the New Jersey National Guard or the reserve component of the United States armed forces; and
      3. has been ordered to active service in the United States armed forces in time of war or national emergency, resulting in the student’s relocation.
    5. The student is kept in the home of a person domiciled in the school district who is not the student’s parent or guardian, and the parent/guardian:
      1. is a member of the New Jersey National Guard or the reserve component of the United States armed forces; and
      2. has been ordered into active military service in the United States armed forces in time of war or national emergency.
    6. The student resides on federal property.

McKinney-Vento Education of Homeless Children and Youth (McKinney-Vento EHCY) program provides students experiencing homelessness with protections and services to ensure they enroll in and attend school; complete their high school education and be positioned for success after graduation, so they may avoid poverty and homelessness as adults.
