Student Registration

| Registration will consist of three steps. You must complete steps 1-2, including the online pre-registration, and locate all required documents prior to attending your in-person appointment (step 3), or the registrar will not be able to assist you at that time. |

IMPORTANT: Write down the username and password when creating your parent/guardian account. This way you can log back in to complete the application if need be!
STEP 2:
Locate the documents listed below. It is highly recommended that all of these documents be uploaded to your application. Regardless if they are uploaded or not, all documents MUST be brought to your registration appointment or Registration Fair.
For the PSD Medical, Physical, McKinney Vento Form (if needed) and Residency Affidavit (if needed), you can find them by clicking the link below! These will be available to fill out at your registration appointment as well.
- Parent/Guardian Driver's License or valid Photo ID
- Certified birth certificate with raised seal. (Even after uploaded, the original must be brought to your appointment/fair.)
- Deed or Lease
- You only need the Notarized Affidavit if you do not have a written Deed or Lease
- Proof of Residency #1
- Proof of Residency #2
- PSD Medical Form
- Physical Form filled out by a Physician
- Immunization Records
- Custody Documents (if any)
- Previous School Records (if any)
- Previous IEP Documents (if any)
- McKinney Vento Forms (if needed)
